The four designated supports listed below require the submission and approval of an Accommodation Request Form. The Accommodation Request Form can be found under the Administration and Security Forms module in the Test Information Distribution Engine (TIDE) and must be submitted by the due dates listed in the Calendar of Events.

• Complex Transcribing

• Mathematics Scribe

• Other
